  • 描述: 步態恢復是下肢障礙病患在復健上之重要指標。現今臨床的步態訓練原理,大多採取動作學習理論,該理論認為學習具有作業特定性,只有針對欲學習的動作不斷地做練習,才易達到最佳的效果,當動作本身具有功能性的意義時,也較容易有學習效果。透過不斷練習與正常動作相似的功能性活動,是對神經損傷患者最有效的復健方式。目前臨床治療方式,多數是病患由治療師引導,於平坦地面練習走路進行步態訓練,但發現病患步態訓練初期的身體狀況較差、學習意願低,治療師必須花極大的力氣支撐病患的身體重量,以致於治療師難以在訓練過程中矯正病患腳步細微的動作,因而往往容易造成病患不正確的步態,對治療的黃金前三∼六個月來說,往往延遲了獨立步行的時機,故並不完全符合現今臨床的需求。下肢障礙患者運用FES進行復健,已證實能有效的增進及改善患者下肢功能,惟目前卻無法讓病患實現正常步態的表現,有鑑於此,運用FES之特性搭配步態訓練系統,並透過不斷練習與正常動作相似的功能性活動,使神經損傷患者達到最有效及貼近日常行為的復健方式,故本研究研發一功能性電刺激輔助步態訓練控制器,使電刺激與步態軌跡同步進行步態復健,期望能建立更有效促進病患療效的復健模式。以步態訓練而言,結合FES特色,讓物理治療師更能瞭解病患步態的分析功能,才算真正完備。因此,本研究目的在研究使用者的真正需求,再從問題定義、解決方法、概念設計到規劃設計流程進而改良設計出一具適用性之醫療步態訓練系統。
    Gait recovery is an important indicator in rehabilitation for patients with disabled lower limbs. Most current clinical gait training techniques adopt the Motor Learning Theory, which states that best therapeutic results can be achieved through continuous motion, particularly when the motion itself has functional meaning. Functional daily motion is the most efficient way to rehabilitate patients with neurological damage. Gait training is a traditional clinical treatment whereby physical therapists guide patients to walk on flat ground. However, therapists must often support the patient’s weight, which is difficult given the patient’s loss of strength, unstable physical condition, and lack of motivation to learn. Particularly in the critical treatment stage of the first three to six months, therapists have difficulty in correcting some small movements that may potentially lead to a patient’s independent walking. Therefore, current gait training does not meet the needs of effective clinical treatment.Patients using functional electrical stimulation (FES) have been able to improve motor function in the lower limbs, even though they cannot fully recover the full motor function. Therefore, using FES combined with gait training, through the continuous mimicking of normal daily motion, should help spinal cord injury patients to achieve higher efficacy in rehabilitation and increase in quality of life. This paper researches the control device necessary to combine FES and gait training. With the ability to combine the advantages of FES with gait training, this research shall hopefully be useful in the development of future treatment models. Physical therapists can use this control device to further their understanding of human gait characteristics. The purpose of this research is to provide researchers with the ability to identify issues, propose solutions, and design new treatment concepts to ultimately create novel and highly effective treatment models.
